Be sure to bring the requested documentation to your appointment. You can register a fictitious name online at dos.pa.gov.
1. Fictitious Name Certificate is applicable if the owner’s last name is not included in the business name.

  • Government ID
  • SSN (if DBA) or EIN Certificate
  • Fictitious Name Certificate1, if applicable
